The Lead Remote Patient Monitoring (RPM) Home Telehealth (HT) Care Coordinator is registered nurse with advanced knowledge in clinical care, education, care coordination, care transition, virtual care modalities and technologies, quality improvement, patient safety, and nursing practice standards. The Lead RPM-HT Care Coordinator assists the Telehealth Assistant Nurse Manager (ANM) and the Facility Telehealth Coordinator (FTC) also known as the Telehealth/Rural Health Nurse Manager in providing programmatic, educational, performance improvement and clinical support. He/she is accountable for adhering to established policies, standards of care, evidence-based practice, national directives, program operation manuals and applicable statutes in nursing care. The Lead RPM-HT Care Coordinator assumes accountability for patient outcomes, clinical competencies, evidenced-based practice, performance improvement, department education, and patient satisfaction. He/she provides direct care utilizing all aspects of the nursing process, care management principles and technologies within a collaborative, interdisciplinary practice setting. Additionally, the Lead RPM-HT Care Coordinator will be a leader for improving patient care, identifying improvement opportunities and enhancing staff education and professional development using evidenced-based practice. The Lead RPM-HT Care Coordinator is involved in improving healthcare through the process of quality assurance, quality improvement and resource utilization by facilitating solutions that are feasible, cost effective and sustainable. The lead RPM-HT Care Coordinator will function as the lead and master preceptor for RPM-HT and participate in Local, VISN and National Connected Care workgroups.
